When is nail surgery recommended for nail fungus?

Fungal nail infection can make a nail thick, brittle, discoloured, or partly detached. Surgery does not by itself remove fungus from surrounding skin or the nail-growing area, so clinicians usually assess the diagnosis and discuss medical treatment as well. Removing part or all of a nail may be considered when the nail is causing pressure, has substantial structural damage, or prevents medicine from reaching the affected area. A sample or other examination may be useful because injury, psoriasis, and other nail conditions can look similar.

Arrange an assessment before asking for nail removal. Keep feet clean and dry, change damp socks, use footwear that does not press on the nail, and avoid sharing nail tools. Do not apply leftover medicines without confirming the diagnosis. Tell the clinician about liver problems, other medicines, diabetes, circulation concerns, and previous treatments. Ask whether trimming, filing, topical medicine, oral medicine, or surgery best fits the nail and how recurrence will be reduced.

Seek prompt care for increasing pain, spreading redness, warmth, pus, fever, or a wound that is not healing. People with diabetes, poor circulation, reduced sensation, or impaired immunity should obtain advice early for any nail or foot wound. After surgery, report bleeding that persists, worsening swelling, foul drainage, or new fever to the treating clinic.

Ask how the diagnosis was confirmed, why surgery is being considered, whether the nail matrix is involved, what additional antifungal treatment is needed, and how long healing may take.
