When is nail surgery recommended for an ingrown toenail?

An ingrown toenail occurs when the nail edge presses into nearby skin, commonly at the big toe. Mild cases may settle with roomy footwear, careful nail care, and treatment selected after examination. Surgery may be discussed when the nail edge repeatedly grows into the skin, inflammation interferes with walking, or drainage and granulation tissue continue. A clinician may remove only the affected edge or address more of the nail, depending on the pattern and recurrence. The aim is to relieve pressure and help prevent the same edge from growing back.

Have the toe examined before attempting to cut deeply into the corner. Wear open or wide footwear, keep the area clean and dry, and avoid picking at the skin. Tell the clinician about diabetes, reduced circulation, nerve problems, immune conditions, or medicines that affect healing. Before surgery, ask how much nail will be removed, how the toe will be dressed, when normal footwear is suitable, and what care is needed during healing.

Arrange prompt medical care for spreading redness, increasing warmth or swelling, pus, fever, worsening pain, or difficulty walking. People with diabetes, poor circulation, reduced sensation, or a weakened immune system should seek advice early because a small toe wound may need closer monitoring. After surgery, contact the clinic for persistent bleeding, worsening swelling, or discharge rather than trying to manage these changes alone.

Ask whether the nail edge can be managed without surgery, what is driving recurrence, whether infection is present, how pain will be controlled, and which changes require a follow-up visit.
