When is joint aspiration recommended for olecranon bursitis?

The olecranon bursa is a small fluid-filled sac at the back of the elbow. A clinician uses a needle to remove a sample, which can be examined for signs of infection or crystal deposits. Removing fluid may also reduce pressure when the swelling is large or uncomfortable. Aspiration is not automatically needed for every case, especially when the swelling is mild and there are no signs suggesting infection.

Arrange an assessment if the elbow remains swollen, becomes more painful, or interferes with leaning on the arm. Tell the clinician about skin breaks, fever, medicines that affect bleeding, and previous episodes. After aspiration, keep the dressing clean, follow activity advice, and watch the puncture site. Do not try to drain the bursa yourself, because this can introduce infection.

Seek prompt medical care for a rapidly enlarging, hot, red, or very tender elbow, drainage from the skin, fever, chills, or feeling unwell. These features can occur with an infected bursa and need timely assessment. Urgent review is also sensible if pain and swelling worsen quickly or arm movement becomes markedly limited.

Ask whether the fluid should be tested, what findings would change treatment, how long to protect the elbow, and what symptoms should prompt a return visit.
