When is joint aspiration recommended for gout?

Gout is caused by urate crystals irritating a joint. A needle can remove a small amount of joint fluid for examination. Finding crystals supports gout, while checking the fluid for infection is important because infection can produce similar pain, warmth, and swelling. Aspiration may also reduce pressure in a very tense joint. Blood urate results alone do not always settle the cause of an acute attack.

Contact a clinician promptly if a joint becomes suddenly painful, swollen, and warm, especially if this is your first episode or the pattern has changed. Share your medicines, kidney history, recent procedures, and any fever. Do not stop prescribed gout treatment without advice. Rest the joint, avoid dehydration, and use only pain medicine that is appropriate for your health conditions.

A hot swollen joint with fever, chills, marked illness, or rapidly worsening pain needs urgent assessment because infection must be excluded. Seek emergency help for confusion, fainting, severe weakness, or breathing difficulty. After aspiration, report increasing redness, drainage, bleeding, or pain at the puncture site.
