When Is Imiquimod Treatment Recommended for Actinic Keratosis?

Actinic keratosis is a rough or scaly change in sun-damaged skin. Imiquimod is a topical medicine that stimulates a local immune response. It may be useful when several visible or less obvious areas sit within one sun-damaged field, rather than when a single lesion clearly needs a procedure or laboratory confirmation.

A rough patch is not automatically actinic keratosis. A thick, tender, bleeding, rapidly growing, or persistent lesion may need examination and sometimes sampling to rule out a more advanced skin cancer before cream treatment is chosen.

Use imiquimod only on the schedule and skin area prescribed. Wash hands before and after application, avoid eyes and mucous membranes, and do not cover the area unless instructed. Redness, scaling, crusting, soreness, or inflammation can occur as the immune response develops.

Do not add other irritating products or treat extra areas without checking first. Tell the prescriber about pregnancy, breastfeeding, immune problems, medicines, and previous reactions. Sun protection and regular skin checks help reduce further ultraviolet exposure and identify new changes.

Contact the prescriber if the reaction becomes severe, spreads beyond the treated field, interferes with eating or sleeping, or does not settle after the instructed treatment period. Seek prompt assessment for a lesion that becomes very thick, ulcerated, persistently painful, or repeatedly bleeds.
