When is fracture fixation surgery recommended for an ankle fracture?

An ankle fracture may involve the outer ankle bone, inner ankle bone, the back of the shin bone, or more than one area. The clinician checks the skin, swelling, circulation, sensation, and ability of the joint to remain aligned. X-rays and sometimes other imaging show whether the fracture enters the joint or affects ankle stability. A well-aligned stable fracture may be treated with a boot or cast. Surgery is more likely when alignment is lost, the joint surface is disrupted, the ankle is unstable, the bone has broken through the skin, or a non-surgical approach cannot hold the position.

Until the plan is clear, keep weight off the injured ankle as instructed, elevate it, and use the support provided. Do not adjust a splint or try to straighten the ankle. Before surgery, discuss medicines, eating and drinking instructions, and transport home. After fixation, protect the wound and follow weight-bearing limits carefully. Rehabilitation commonly builds from swelling control and safe movement toward strength, balance, and walking, guided by follow-up findings.

Get emergency help for an ankle that looks newly deformed, an open wound over the fracture, severe pain with tight swelling, or toes that are cold, pale, blue, numb, or difficult to move. After surgery, seek prompt advice for fever, spreading redness, increasing drainage, wound separation, calf swelling with pain, chest pain, or sudden breathlessness.

Ask whether the fracture is stable, whether the joint surface is involved, how long you must avoid weight bearing, and what fixation is planned. Clarify wound care, blood-clot warning signs, follow-up imaging, driving, work, and the milestones for physiotherapy.
