When is fracture casting recommended for a scaphoid fracture?

The scaphoid is a small wrist bone that helps connect wrist movement. Pain near the thumb side of the wrist can follow a fall, and early imaging may not show every fracture clearly. The clinician uses the examination and appropriate imaging to judge the fracture’s location, alignment, stability, and blood supply. A cast that includes the wrist and often the thumb may protect the bone. A displaced fracture, a break in a higher-risk area, delayed healing, or uncertain diagnosis may lead to closer review or surgery instead.

Wear the cast for the full period advised, even if wrist pain improves. Keep it dry, support the hand when resting, and move the uncovered fingers to limit stiffness if permitted. Avoid lifting, gripping, pushing, and contact activities until cleared. Do not test the wrist or press over the tender area. Attend repeat appointments because symptoms can settle before the scaphoid is strong enough for normal use.

Seek urgent care for worsening or severe pain, new numbness, pale or blue fingers, marked swelling, or fingers that feel cold or cannot move normally. Contact the treating service if the cast cracks, becomes loose, causes pressure, gets wet, or develops an odour or drainage. Return for reassessment if thumb-side wrist pain continues after an apparently minor fall.

Ask where the fracture sits, whether the cast includes the thumb, how healing will be checked, and which activities are restricted. Clarify what symptoms should trigger an earlier review and when wrist movement or physiotherapy can begin.
