When is fracture casting recommended for a growth plate fracture?

A growth plate is a developing area near the end of a child’s bone. The clinician considers the child’s age, the injured body region, pain and swelling, skin condition, and imaging. Some growth plate injuries are not obvious on an early image, so the examination and follow-up plan matter. A cast can limit movement and protect alignment. If the bone is displaced, the joint is affected, the skin is open, or alignment cannot be maintained, the child may need reduction, specialist monitoring, or another treatment.

Help the child keep the cast dry, avoid inserting toys or fingers underneath it, and elevate the limb when swelling is present. Encourage movement of uncovered fingers or toes if the clinician allows it. Follow restrictions on running, climbing, sports, and weight bearing. Keep every review appointment: healing near a growth plate is checked for alignment and ongoing development, not only for pain relief. Never trim or remove the cast at home.

Seek urgent care if the child has worsening pain despite the advised medicine, increasing tightness, new numbness, pale or blue fingers or toes, unusual coldness, or difficulty moving them. Contact the treating team for a wet, cracked, loose, or foul-smelling cast, fever, drainage, or swelling that keeps increasing.

Ask which growth plate is involved, whether alignment is stable, which activities are restricted, how the cast should be protected at school, and when repeat imaging is needed. Ask how the team will monitor bone growth after the cast comes off.
