When is surgical excision recommended for xanthelasma?

Xanthelasma consists of yellowish cholesterol-rich deposits, often near the inner eyelids. A clinician first confirms that the appearance fits xanthelasma and checks the eyelid skin and nearby structures. Excision removes the deposit through a carefully placed incision, but the eyelid has limited skin and a visible scar or a change in contour is possible. Deposits can return, particularly when underlying lipid problems are not addressed.

Arrange an assessment for a new or changing eyelid plaque, especially if it is one-sided, inflamed, ulcerated, or affecting eyelid movement. Ask whether a lipid test or another evaluation is appropriate, since xanthelasma can occur alongside cholesterol disorders but does not establish one by itself. Discuss excision, other local treatments, recurrence, scar placement, and protection of the eye during treatment.

Seek prompt review for eyelid swelling with pain, vision change, discharge, ulceration, bleeding, or rapid alteration in shape. After removal, report worsening swelling, severe pain, vision disturbance, spreading redness, pus, fever, or an eyelid that cannot close normally.

Ask how the diagnosis was established, whether the deposit reaches the eyelid margin, and which removal method best protects eyelid function. Clarify anaesthesia, scar care, the chance of return, lipid evaluation, recovery time, and what symptoms require urgent eye assessment.
