When is surgical excision recommended for dermatofibroma?

A dermatofibroma is a firm skin nodule that commonly feels anchored in the skin and may look darker than nearby skin. If it has a typical appearance and causes no problems, observation may be reasonable. Excision can be considered when repeated rubbing causes pain or irritation, when the nodule catches on clothing, when it changes in appearance, or when examination cannot establish a confident diagnosis. The entire nodule can then be sent for laboratory assessment. Because the lesion may extend deeper than it looks, removal can leave a scar and may not be advised solely for cosmetic reasons.

Have the nodule examined before deciding on removal, particularly if it is new or evolving. Tell the clinician about tenderness, bleeding, repeated trauma, and any change in colour or firmness. Discuss the likely scar, wound closure, local anaesthetic, and whether a small sample or complete removal is more appropriate. Follow dressing and bathing instructions after a procedure and avoid pulling at the healing wound.

Seek a timely skin assessment if the nodule grows, becomes persistently painful, bleeds, ulcerates, or develops a markedly different colour or border. After excision, report increasing swelling, pus, spreading redness, wound separation, or bleeding that does not stop with pressure.

Ask what makes the lesion look like a dermatofibroma, whether a biopsy is needed, how deep excision may be, and what result would change the follow-up plan.
