When is diagnostic laparoscopy recommended for chronic pelvic pain?

Chronic pelvic pain can arise from gynaecological, bowel, bladder, muscle or nerve-related problems. A laparoscopy uses a camera through a small abdominal incision to inspect the pelvis. It can identify visible endometriosis, scar tissue, ovarian or tubal changes and some other abnormalities that scans may not show clearly.

Laparoscopy is surgery, so the decision balances possible diagnostic value with anaesthesia, recovery and surgical risks. Sometimes treatment, such as taking a biopsy or treating visible endometriosis, can be discussed, but this should be agreed in advance.

Bring a pain diary describing timing, cycle relationship, bowel or bladder symptoms, sex-related pain and treatments already tried. Ask what non-surgical assessments remain, what the camera may not detect and whether treatment could happen during the same operation.

Tell the surgical team about pregnancy possibility, allergies, previous abdominal surgery and medicines that affect bleeding. Plan transport and recovery support because tiredness and abdominal discomfort can follow anaesthesia.

Seek urgent care for sudden severe pelvic pain, fainting, heavy bleeding, fever or vomiting, especially if pregnancy is possible. After laparoscopy, contact the surgical team for increasing pain, fever, wound redness, breathing difficulty or inability to keep fluids down.
