How orthotics can help

Hallux rigidus affects the joint at the base of the big toe. Bending the toe during push-off may become painful, and shoes can press on the joint. An orthotic can be designed to reduce bending at that joint or spread load away from a tender area. The assessment may include joint movement, walking, footwear and whether the pain comes from the top of the joint, underneath the foot or elsewhere. An orthotic manages forces; it does not remove a joint change.

Practical steps

Choose shoes with a wide toe box and a sole that does not force the big toe to bend sharply. A clinician may recommend a stiff sole, a modified insert or a custom device depending on your walking pattern. Introduce an orthotic gradually and check the skin around the big-toe joint after use. Keep a note of which activities trigger pain, because this helps the clinician adjust the device and plan exercises or other treatment.

Book an evaluation when pain persists, the joint becomes increasingly stiff, footwear repeatedly irritates the area or you begin changing how you walk. Seek prompt care for sudden severe pain with redness and warmth, an injury with swelling, inability to bear weight or new numbness. A timely examination can distinguish joint irritation from other causes of forefoot pain.
