When is custom orthotics recommended for bunions?

Short answer

Custom orthotics may be recommended when a bunion causes pressure, altered foot loading, or difficulty finding comfortable shoes and simpler measures are insufficient.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Main benefit sought
Less rubbing or more even pressure inside a suitable shoe
Key limitation
An insert does not straighten a fixed bunion
First footwear check
A wide, deep toe box without side pressure

What an orthotic can do

A bunion, or hallux valgus, is a change in alignment around the big-toe joint. It can create rubbing at the side of the foot, crowd the toes, and alter how weight moves across the sole. An assessment looks at the deformity, flexibility, skin pressure, footwear, walking pattern, and any pain under the forefoot.

A custom orthotic may cushion a pressure area or redistribute load inside a suitable, roomy shoe. It will not straighten a fixed deformity or make narrow footwear safe. Explore [Bunions](/conditions/bunions/) and [Custom Orthotics](/treatments/non-surgical-orthopedics/custom-orthotics/).

Practical steps

Choose shoes with a wide, deep toe box and avoid shapes that press the big toe inward. Bring the shoes you wear most often to the fitting appointment. Introduce the device gradually and inspect the skin for redness, blisters, or pressure marks, especially if sensation is reduced.

Use the orthotic together with footwear changes, toe and foot exercises if advised, and activity pacing. If discomfort comes from the bunion rubbing rather than load under the foot, a wider shoe or protective pad may be more useful than an insert. The clinician can reassess the design when comfort or walking does not improve.

When to seek care

Arrange medical review for a painful open sore, spreading redness, warmth, drainage, or swelling around the bunion. Seek prompt assessment after an injury that causes sudden deformity or inability to bear weight. Numbness, a colour change, or persistent pressure from the device warrants fitting advice before skin damage develops.

Questions for your doctor

Is my bunion flexible enough for an insert to help? Where is pressure concentrated during walking? Would footwear, a pad, or a ready-made insert be a better first step? What signs show that the orthotic is irritating my skin or changing my gait?

Questions people ask

Can custom orthotics remove a bunion?

No. They may improve pressure distribution and comfort, but they do not remove or reliably straighten the bony alignment.

Are orthotics useful if shoes rub the side of my foot?

They can help in selected cases, but a roomy toe box and reduced side pressure may matter more when rubbing is the main problem.
