When is cortisone injection recommended for rotator cuff tear?

A tear affects one or more tendons that help move and stabilise the shoulder. Cortisone can reduce inflammation around the painful area, which may create a window for comfortable movement and physiotherapy. It does not stitch the tendon back together or restore strength by itself. The decision depends on the tear’s size and location, weakness, activity needs and previous treatment.

Before an injection, describe which movements hurt, whether weakness began suddenly, and what you have already tried. Follow the advised activity restriction after the procedure, then progress shoulder exercises gradually. Avoid using reduced pain as permission for heavy lifting. A rehabilitation plan remains central, and persistent weakness may require review by an orthopaedic clinician.

Arrange prompt assessment after a fall or lifting injury if you cannot raise the arm, notice sudden marked weakness or see a new shoulder deformity. Seek urgent help for severe swelling, fever, spreading redness or drainage after an injection. These signs need assessment rather than another pain treatment.

Ask what the examination or scan shows, whether the injection could affect tendon healing, how it fits with physiotherapy, and what alternatives address weakness as well as pain.
