When is chemotherapy recommended for Waldenstrom macroglobulinaemia?

Waldenstrom macroglobulinaemia is a blood cancer involving lymphoplasmacytic cells that produce an abnormal protein called IgM. If a person has no symptoms and stable tests, the team may monitor rather than start treatment immediately. Treatment is considered when the disease causes problems such as anaemia, troublesome bleeding, enlarged lymph nodes, organ enlargement, nerve symptoms, kidney problems, or thickened blood that affects vision or the nervous system.

Chemotherapy is one possible part of treatment. It may be paired with an antibody medicine or another targeted drug, depending on previous treatment, symptoms, test results and general health. When blood becomes too thick, plasma exchange may be needed urgently to remove IgM while longer-term treatment begins. This procedure does not replace cancer treatment.

Keep appointments for blood counts, IgM measurement and assessment of symptoms even during observation. Tell the haematology team about headaches, blurred vision, dizziness, unusual bleeding, worsening numbness or shortness of breath. Ask which treatment goal applies to you, how quickly improvement is expected, and how infection risk, blood counts and nerve symptoms will be followed.

Seek urgent medical help for sudden vision change, severe headache, confusion, weakness, fainting, chest pain or difficulty breathing. Contact the treatment team promptly for fever, chills, bleeding, severe diarrhoea, repeated vomiting or a new infection symptom while receiving chemotherapy.
