When is chemotherapy recommended for uterine sarcoma?

Uterine sarcoma is a group of cancers that arise from muscle or connective tissue in the uterus. Leiomyosarcoma and endometrial stromal sarcoma can behave differently, so the pathology report guides treatment. The team considers tumour size, grade, subtype, spread, surgical margins and whether cancer cells are found outside the uterus. Chemotherapy after surgery may be discussed when these findings suggest a substantial chance of hidden disease. It can also be used when the cancer returns or cannot be removed fully.

For some people, chemotherapy is combined with surgery or radiotherapy; for others, another systemic medicine or observation may fit better. The aim may be to reduce recurrence risk, shrink cancer before an operation, or control disease and symptoms. The likely benefits and side effects should be weighed against personal priorities.

Request a clear explanation of the sarcoma subtype and stage, and ask whether a specialist pathology review is useful. Discuss the purpose of chemotherapy, the proposed medicines, treatment setting, monitoring plan and alternatives. Tell the team about heart, kidney or liver problems, fertility concerns, medicines and any plans for pregnancy. A written schedule can help you prepare for appointments and laboratory checks.

During treatment, report fever, chills, persistent diarrhoea, repeated vomiting, mouth sores that prevent drinking, unusual bleeding or worsening pelvic pain.
