When is chemotherapy recommended for soft tissue sarcoma?

Soft tissue sarcoma includes different tumour types, and their sensitivity to chemotherapy varies. The oncology team considers the pathology report, grade, size, position, whether the tumour can be removed, and whether scans show disease beyond its original site. Chemotherapy before surgery may be discussed when shrinking or controlling a tumour could make local treatment more manageable. After surgery, it may be considered when features suggest a higher risk of cancer returning. For disease that cannot be removed or has spread, chemotherapy can aim to slow growth, reduce symptoms, or preserve day-to-day function.

Ask for a clear explanation of the sarcoma subtype and the purpose of treatment. Discuss whether surgery, radiotherapy, targeted medicines, or observation also have a role. Before treatment, review the planned medicines, infusion schedule, blood-test monitoring, fertility considerations, and possible effects on nerves, blood counts, digestion, or hair. Tell the team about existing conditions and all medicines or supplements.

Contact the oncology team promptly for fever, chills, new breathlessness, chest pain, uncontrolled vomiting, severe diarrhoea, confusion, or bleeding during treatment. A rapidly enlarging lump, worsening pain, new weakness, or loss of function also needs timely assessment, even if chemotherapy has not started.
