When is chemotherapy recommended for prostate cancer?

The decision depends on where the cancer is, how quickly it is changing, previous treatments, symptoms, blood test trends, scan findings and your fitness for treatment. Chemotherapy can be combined with hormone treatment in some advanced settings, or used when cancer continues despite hormone-based medicines. It is not routinely needed for every newly diagnosed prostate cancer. The aim may be to slow growth, relieve symptoms, preserve function or support control of disease. Your clinician will balance expected benefit with effects such as tiredness, infection risk, appetite changes and nerve symptoms.

Bring a list of medicines and supplements, and mention diabetes, heart problems, nerve symptoms or previous reactions to treatment. Ask what the goal is, how chemotherapy fits with hormone therapy, how it will be given, and how side effects will be monitored. During treatment, call your oncology team urgently for fever or chills, new breathlessness, persistent vomiting, severe diarrhoea, confusion or marked weakness. New leg weakness, loss of bladder or bowel control, or severe back pain needs emergency assessment because prostate cancer can affect the spine. Do not wait for a routine appointment when these symptoms appear.
