Short answer

Chemotherapy may be recommended for metastatic breast cancer when rapid control is needed, symptoms are affecting daily life, or other treatments are not suitable or no longer controlling the disease.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Treatment aim
Control metastatic disease, relieve symptoms and protect quality of life.
Decision factors
Cancer features, previous treatment, symptoms, health and preferences.

How the decision is made

Metastatic breast cancer has spread beyond the breast or nearby lymph nodes. Treatment is usually planned as an ongoing strategy to control the cancer, ease symptoms and support quality of life. Your oncology team considers the cancer's hormone-receptor and HER2 results, where it has spread, how quickly it is changing, medicines you have already received, your general health and your preferences.

Chemotherapy can be given alone or alongside another cancer treatment. It may be chosen when a tumour is growing quickly, when organ function is threatened, or when targeted or hormone-based treatment is unlikely to help. The plan can change if scans, symptoms or side effects change.

Preparing for a treatment discussion

Bring a list of current medicines, supplements, allergies and previous cancer treatments. Ask which benefit the team expects, how treatment will be delivered, how often appointments occur, and how symptoms will be monitored. Discuss fertility, pregnancy, work, travel and support at home when relevant.

Before treatment, the team may check blood counts, kidney and liver function, and other findings that affect drug selection. Report new symptoms promptly and keep a record of fever, pain, appetite, bowel changes or numbness so the team can adjust supportive care.

When to seek urgent help

Contact your oncology service urgently for a fever or chills during treatment, especially if you have been told that infection risk is increased. Seek emergency care for severe breathing difficulty, chest pain, confusion, fainting, uncontrolled bleeding or sudden weakness. New severe back pain, leg weakness, or loss of bladder or bowel control also needs immediate assessment because spinal or nerve pressure can require urgent treatment.

Questions for your doctor

Which features of my cancer make chemotherapy appropriate now? What other treatment options fit my results? How will we judge whether it is helping? Which side effects should I report the same day, and whom should I call outside clinic hours?

Questions people ask

Does needing chemotherapy mean treatment has stopped working?

Not necessarily. Chemotherapy can be one part of a planned sequence, or it may be introduced when another treatment is no longer controlling the cancer.

Can I discuss side-effect support before starting?

Yes. Ask about medicines, monitoring and practical help for fatigue, nausea, infection risk, mouth changes or nerve symptoms before the first dose.
