When is chemotherapy recommended for bowel cancer?

After an operation, chemotherapy can treat cancer cells that may remain beyond the area removed. This is called adjuvant treatment. For some locally advanced tumours, treatment before surgery can shrink the cancer and help plan the operation. If bowel cancer has spread to another organ or cannot be removed with surgery, chemotherapy may be used to slow or control it, ease symptoms, or make another treatment possible.

The decision can take account of the tumour's location, stage, features seen in the laboratory, whether lymph nodes are involved, previous treatment, kidney and liver function, and how well you are able to manage treatment. Chemotherapy may be given alone or alongside another cancer treatment.

Ask which treatment goal applies to you and whether chemotherapy is planned before surgery, after surgery, or as treatment for advanced disease. Your team can explain the proposed medicines, how they are given, likely effects on daily life, and how symptoms will be monitored. Tell them about other medicines, supplements, bowel symptoms, numbness, allergies, and any concerns about eating, work or travel.

A treatment plan may change if scans, laboratory results or your recovery provide new information. Keep a record of fever, diarrhoea, vomiting, reduced urination or new pain while receiving treatment, and use the contact number supplied by your oncology team.

Contact your oncology team urgently for a fever or chills during chemotherapy, repeated vomiting, severe or worsening diarrhoea, inability to drink, unusual bleeding, breathlessness, confusion, or pain that is not controlled. Get emergency help for severe breathing difficulty, collapse, sudden weakness, or heavy bleeding. These symptoms need prompt assessment and are not a way to judge whether chemotherapy is working.
