When is chemotherapy recommended for anal cancer?

For many localised anal cancers, chemotherapy is given alongside radiotherapy as chemoradiation. The medicines can make cancer cells more sensitive to radiation, helping treatment work on the anal tumour and nearby lymph nodes. Surgery is not usually the first treatment when chemoradiation is suitable, but an operation may be considered if cancer remains or returns. If the cancer has spread beyond the original area, chemotherapy may be used as a systemic treatment, sometimes with other medicines, according to the cancer type and treatment plan.

Your oncology team will review biopsy findings, scans, kidney function, blood counts, hearing, other health conditions and medicines before recommending a regimen. Tell them about pregnancy, fertility concerns, numbness, bowel symptoms and any previous cancer treatment. Ask how chemotherapy will be scheduled with radiotherapy, how side effects will be managed, and whether you need a central line. Keep a record of temperature, bowel changes, pain and skin irritation during treatment, and report changes promptly.

Contact your cancer team urgently for fever or chills, severe diarrhoea, repeated vomiting, inability to drink, new confusion, unusual bleeding, or worsening pain and redness around treated skin. Seek emergency help for trouble breathing, fainting, sudden weakness or heavy bleeding. These symptoms can need prompt assessment during chemotherapy and radiotherapy.
