When is caesarean section recommended for placenta praevia?

The placenta may be low in the uterus, sometimes over the opening between the uterus and vagina. As the cervix changes for labour, placental blood vessels can bleed. The maternity team uses ultrasound findings, bleeding history, pregnancy stage and the baby's condition to plan birth. If the placenta moves farther from the cervix as pregnancy progresses, the recommended route may change.

A planned caesarean allows the team to prepare blood products, anaesthesia and newborn support when needed. Heavy or continuing bleeding, labour beginning unexpectedly, or signs that either patient or baby is becoming unwell can require an earlier birth.

Keep the recommended ultrasound appointments so the placental position can be reassessed. Tell the maternity unit about any vaginal bleeding, contractions, pelvic pressure or fluid loss, even if bleeding settles. Follow personalised advice about activity, sex and travel, because restrictions depend on bleeding and scan findings. Contact your maternity unit urgently for any vaginal bleeding in pregnancy, particularly bright-red bleeding, bleeding with pain or contractions, dizziness, faintness or reduced fetal movement. Call emergency services for heavy bleeding, collapse, severe weakness or difficulty breathing. Do not drive yourself if you feel faint. A lack of pain does not make bleeding safe to ignore.

Ask where the placenta is in relation to the cervix, whether a later scan is needed, what symptoms should bring you to hospital, and what circumstances could move the birth plan earlier. Ask when the caesarean is expected and how anaemia, blood availability and recovery will be managed.
