When is brachytherapy recommended for prostate cancer?

Brachytherapy places radioactive sources in or near the prostate so radiation can focus on the treatment area. It can be delivered as temporary high-dose-rate treatment or as small permanent seeds, depending on the plan. Doctors review the cancer grade, PSA pattern, scans, prostate size, urinary symptoms and whether disease is outside the prostate. Some people receive it alone; others have it alongside external-beam radiation or hormone treatment. A large prostate, significant obstruction or troublesome urinary symptoms may need management before an implant, or may make another approach more appropriate.

Ask a radiation oncologist and urologist to review your biopsy, imaging and urinary history together. Assessment may include a physical examination, prostate imaging and a discussion of anaesthesia, catheter use, bowel symptoms, sexual function and follow-up PSA testing. Tell the team about medicines that affect bleeding, previous prostate procedures and any difficulty passing urine. Do not stop a prescribed medicine without instructions. The choice should compare brachytherapy with surgery, external-beam radiation, active surveillance or other options that fit your risk group and priorities.

Contact your cancer team promptly for inability to pass urine, heavy bleeding, fever, worsening pelvic pain or new severe weakness after the procedure. Seek emergency help for sudden confusion, collapse, severe breathing difficulty or uncontrolled bleeding. Before treatment, arrange urgent review for rapidly worsening urinary obstruction or blood in the urine with clots, because these problems can affect safe planning.
