When is bone-strengthening therapy recommended for high calcium in cancer?

Some cancers increase the release of calcium from bone into the bloodstream. High calcium can cause thirst, frequent urination, constipation, nausea, muscle weakness, drowsiness, or confusion. Bone-strengthening medicines can slow the bone breakdown that contributes to the calcium rise. They may be used as part of a hospital or oncology treatment plan, often alongside fluids and treatment for the cancer causing the problem.

The medicine is not suitable for everyone. Kidney function, hydration, calcium and vitamin levels, dental health, and previous bone treatments all influence the choice and timing. Blood tests help the team judge response and monitor for unwanted effects.

Follow the treatment team's instructions about fluids and medicines, and tell them about vomiting, diarrhoea, kidney disease, heart problems, or difficulty drinking. Provide a full list of prescription medicines and supplements. Do not start calcium or vitamin supplements unless the treating team advises it. Ask how soon blood tests are needed and whether dental review is appropriate before ongoing bone treatment.

Seek emergency help for new confusion, extreme drowsiness, fainting, severe weakness, repeated vomiting, inability to drink, chest symptoms, or very little urine. Contact the oncology team promptly for increasing thirst, constipation that is not improving, worsening nausea, or muscle weakness. High calcium can worsen quickly and may need treatment in hospital.

Ask what is driving the calcium rise, whether treatment needs to start in hospital, how kidney function affects medicine choice, when calcium will be rechecked, and what symptoms require immediate help.
