When Is a Blood Test Recommended for Gout?

Gout is caused by urate crystals irritating a joint, often producing sudden pain, swelling, warmth and tenderness. A blood test measures uric acid and may assess kidney function, blood counts or inflammation. A uric-acid result alone cannot prove or exclude gout: the level can be lower during an acute flare, and raised uric acid does not mean a person has gout. If the diagnosis remains uncertain, a clinician may discuss examining joint fluid or using imaging.

Arrange an assessment for a hot, swollen joint, especially if this is the first episode. Tell the clinician when symptoms began, which joints are affected, whether you have had kidney stones or kidney disease, and every medicine or supplement you take. Do not change gout medicine based on one result without advice. During follow-up, repeat testing may help guide urate-lowering treatment, while kidney checks can help determine which medicines are appropriate and how they should be monitored.

Seek urgent medical care for a painful swollen joint with fever, chills, feeling very unwell, or rapidly spreading redness because infection can resemble gout. Prompt assessment is also needed after an injury, with severe unexplained pain, or if you cannot use the limb. Do not wait for a routine blood test when symptoms are severe or worsening.

Ask which tests are being ordered and what each result will change. Clarify whether the sample should be taken during or after a flare, whether medicines can affect the result, whether joint-fluid testing is needed, and how kidney results will influence gout treatment. Ask when follow-up testing is useful and which warning symptoms need same-day review.
