When Is Bisphosphonate Therapy Recommended for Vertebral Compression Fracture?

A vertebral compression fracture can occur when a weakened spinal bone collapses. Bisphosphonates slow bone breakdown and may form part of a longer-term plan to protect the rest of the skeleton. They do not immediately restore the compressed vertebra or replace pain control, bracing, safe movement and rehabilitation. The clinician will consider the fracture cause, imaging, bone-density findings, kidney function, calcium and vitamin D, swallowing or reflux problems, and other medicines.

Keep the follow-up appointment that reviews both fracture healing and bone health. Report the level and pattern of pain, changes in height or posture, and any new falls. Ask whether calcium, vitamin D, a bone-density assessment or another fracture-prevention medicine is needed. If an oral bisphosphonate is prescribed, take it exactly as directed and remain upright for the instructed period. Work with physiotherapy on posture, leg strength, balance and safe lifting; avoid exercises that sharply worsen spinal pain.

Seek urgent assessment for new leg weakness, numbness, loss of bladder or bowel control, inability to stand, or severe pain after a fall. Contact your clinician promptly for pain that is worsening rather than settling, repeated falls, or a new band-like chest or abdominal sensation. During bisphosphonate treatment, report swallowing difficulty, severe heartburn, jaw pain, or new thigh or groin pain.

Ask whether the fracture indicates osteoporosis, what tests confirm your fracture risk, and when bone-protective treatment should start. Discuss oral versus infusion treatment, kidney and calcium checks, exercise limits, pain options, dental care, monitoring, and what symptoms mean you should call the clinic.
