When Is Bisphosphonate Therapy Recommended for Paget's Disease of Bone?

Paget's disease causes areas of bone to be rebuilt in a disorganised way. The affected bone may become enlarged, painful or mechanically weaker, even though different people have different symptoms. Bisphosphonates reduce the overactive bone-remodelling process. Treatment decisions often consider bone pain, blood-test results, the location of active disease, hearing or nerve-related concerns, and whether surgery is being planned near an affected bone. An incidental, inactive area may not need medicine immediately.

Arrange an assessment with a clinician familiar with metabolic bone conditions. Bring your symptom timeline, previous imaging and a complete medicine list. Ask which blood tests will track bone activity and whether kidney function, calcium and vitamin D need checking first. Follow the chosen tablet or infusion instructions carefully. Gentle activity can support function, but high-impact exercise may be unsuitable if a weight-bearing bone is painful or deformed; ask for a physiotherapy plan tailored to you.

Seek prompt care for a sudden increase in bone pain, a new deformity, a fall, or difficulty walking. Urgent assessment is needed for new weakness, numbness, loss of bladder or bowel control, sudden hearing change, severe headache, chest pain or major breathing difficulty. While taking treatment, report severe heartburn, swallowing difficulty, jaw pain, or new thigh or groin pain to the prescriber.
