When is biologic therapy recommended for pemphigus vulgaris?

Pemphigus vulgaris is an autoimmune blistering disease that can affect the skin and moist linings of the mouth, eyes, nose, throat, or genitals. Treatment decisions are made by a specialist because open blisters can lose fluid, become infected, and make eating or drinking difficult. The clinician considers confirmed diagnosis, current blister activity, previous immunosuppressive medicines, steroid effects, infections, vaccination history, and other medical conditions. A biologic modifies immune activity; it is not a quick wound dressing and does not make home care unnecessary.

Keep a record of new blisters, mouth sores, swallowing discomfort, eye symptoms, and medicines used. Clean affected skin gently, avoid peeling the roof from intact blisters, and use dressings exactly as advised. Ask about blood tests, infection screening, vaccination timing, infusion or injection arrangements, and how other immune medicines will be adjusted. Never taper corticosteroids suddenly unless the prescribing team gives instructions.

Seek urgent care for trouble breathing or swallowing, inability to drink, severe eye pain or vision change, rapidly spreading blisters, fever, confusion, or signs of infected wounds such as increasing warmth, pus, or worsening pain. Contact the treatment team promptly for a new infection or allergic symptoms after a dose or infusion.

Ask why a biologic fits your current disease activity, what steroid-sparing goal is being pursued, and how response will be monitored. Clarify wound care, mouth care, infection precautions, vaccination planning, infusion reactions, missed appointments, and the symptoms that require same-day assessment.
