When is biologic therapy recommended for nail psoriasis?

Nail psoriasis can cause pitting, separation from the nail bed, thickening, discoloration, crumbling, or tenderness. Because nails grow slowly, improvement can take time even after skin inflammation settles. A clinician checks whether the nail findings are truly psoriasis, since fungal infection, trauma, and other nail disorders can look similar. The assessment also covers skin plaques, scalp disease, joint pain or stiffness, prior treatments, and infection risks.

Photograph affected nails in consistent lighting and note pain, catching, lifting, and tasks that have become difficult. Tell your dermatologist about all creams, tablets, injections, supplements, infections, and vaccination history. Ask whether a nail sample or other test is needed before treatment. Protect nails from repeated trauma, keep them trimmed without cutting the surrounding skin, and follow the prescribed plan rather than picking under the nail.

Arrange prompt assessment for a suddenly painful, swollen, or pus-filled nail fold, a rapidly changing single nail, or dark pigment that extends onto surrounding skin. Seek urgent help for fever with spreading redness, severe swelling, or breathing difficulty. While taking a biologic, report persistent fever, a new infection, or an unusual rash to the treating team.

Ask how severe your nail disease is, whether joint symptoms change the treatment choice, how long nail improvement may take, and how infection or fungus will be excluded. Discuss injection technique, monitoring, missed doses, and whether another treatment should continue while the nail grows out.
