When is antibiotic therapy recommended for paronychia?

Paronychia affects the skin fold beside a nail, causing redness, swelling, warmth, throbbing pain, or tenderness. Pus may collect at the nail edge. A short-lived infection after a small break in the skin can sometimes improve with warm soaks and protection, but a visible collection may need a clinician to release it; antibiotics alone may not clear trapped pus.

The decision depends on whether redness is confined to the nail fold or extending into the finger, whether fever is present, and whether there was a bite, puncture, nail damage, or repeated water and chemical exposure. Long-lasting swelling may have a different cause and needs assessment rather than repeated self-treatment.

Keep the finger or toe clean and dry between brief warm-water soaks, then pat it dry. Protect it from biting, picking, manicures, detergents, and further trauma. Do not pierce the swollen area or cut the cuticle yourself. Remove rings from an affected finger before swelling increases. If prescribed medicine is started, follow the instructions and return for review if pain, swelling, or redness continues to build.

Seek prompt care when redness travels beyond the nail fold, the whole fingertip becomes tense and painful, movement is limited, or pus is increasing. Fever, red streaks, numbness, a deep puncture, a bite, or rapidly worsening swelling needs urgent assessment. People with diabetes, reduced immunity, or poor circulation should seek advice early for a painful infected nail fold.
