When Is an Antenatal Care Package Recommended for Underactive Thyroid in Pregnancy?

Pregnancy changes the body's thyroid hormone needs. An antenatal plan helps your maternity and thyroid care teams check whether treatment remains suitable and whether thyroid blood tests need repeating. Levothyroxine is commonly used for an underactive thyroid, but the dose may need review during pregnancy. Do not stop, restart or change it without medical advice.

Early contact is especially useful if you were diagnosed before pregnancy, have recently changed treatment, have had thyroid surgery, or take supplements containing iron or calcium. These supplements can affect how levothyroxine is absorbed, so your clinician may give timing instructions.

Contact your obstetrician, midwife or thyroid clinician promptly and tell them the date of your positive test, your current medicine and the dose you take. Bring recent thyroid results and a full list of prescribed medicines, vitamins and supplements. Keep taking prescribed treatment unless your clinician tells you otherwise, and attend the blood tests and antenatal appointments they arrange.

Take levothyroxine consistently as directed, usually separately from food and from iron or calcium products when advised. If nausea or vomiting makes it difficult to keep medicine down, report this rather than doubling the next dose.

Seek urgent maternity advice for heavy vaginal bleeding, severe abdominal pain, fainting, chest pain, trouble breathing, a severe headache with vision changes, or noticeably reduced fetal movement later in pregnancy. These symptoms need assessment regardless of your thyroid history.

Contact your care team promptly if you cannot keep thyroid medicine down, accidentally take much more than prescribed, develop marked weakness or palpitations, or have concerns about a missed dose. A clinician can give specific instructions based on your treatment and stage of pregnancy.
