What to know about rotator cuff tear in CrossFit athletes

Short answer
A rotator cuff tear can limit overhead strength and control, so CrossFit athletes need assessment and a gradual return to loading.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Main concern
- Pain or weakness with overhead and pulling movements
- Useful first step
- Stop provocative loading and arrange a shoulder assessment
- Return to training
- Progress through controlled strength before full-speed skills
What it means
The rotator cuff is a group of shoulder tendons that helps centre the upper-arm bone and move the arm. A tear may develop during a forceful lift, repeated overhead work, or a fall, and it can cause pain, weakness, night discomfort, or difficulty controlling a barbell, kettlebell, or ring movement.
CrossFit movements such as kipping pull-ups, snatches, jerks, handstand work, and high-repetition lifting can expose a painful shoulder to speed and load. Pain after training does not by itself show the size or type of tear; an examination helps distinguish tendon injury from bursitis, joint irritation, or referred pain.
What to do next
Pause movements that reproduce sharp pain, sudden weakness, or loss of control. Keep comfortable daily movement rather than holding the arm completely still, and use lower-body or conditioning options only when they do not provoke the shoulder. A clinician may assess strength and range of motion and decide whether imaging is useful.
Rehabilitation commonly starts with comfortable mobility and controlled shoulder-blade and rotator-cuff work, then progresses to resistance, pressing, pulling, and sport-specific drills. Your coach and therapist can adjust range, tempo, grip, and volume. Return to overhead training should follow reliable control and strength, not a calendar date.
When to seek care
Arrange prompt assessment after a fall, a felt tear, or new inability to lift the arm. Seek urgent help for a visibly deformed shoulder, a cold or numb hand, severe swelling, or rapidly worsening weakness. Persistent night pain or symptoms that interfere with dressing, driving, or sleep also deserve clinical review.
Questions for your doctor
Ask which movements are safe now, whether imaging is needed, how rehabilitation will be staged, and what strength or control you should demonstrate before resuming kipping, Olympic lifts, or handstand work.

Questions people ask
Can I keep exercising with a rotator cuff tear?
Often you can maintain pain-free activity, especially lower-body or modified conditioning, but avoid shoulder loading that causes pain, weakness, or loss of control.
Does shoulder pain confirm a tear?
No. Shoulder pain has several possible causes, and examination is needed to assess whether a rotator cuff tear is involved.
