What to know about nappy rash in children

The skin may look red, sore, shiny, or irritated across the nappy area, including the bottom, groin folds, or upper thighs. A child may be unsettled during changing because the skin stings when it is cleaned. Wetness, a tight nappy, diarrhoea, antibiotics, or a new wipe, soap, or cream can contribute. Rash that involves the skin folds and has small separate spots may need assessment for a yeast infection rather than simple irritant rash.

Change wet or soiled nappies promptly. Rinse with lukewarm water or use fragrance-free, alcohol-free wipes, then pat dry rather than rubbing. Give short nappy-free periods when practical, and fasten nappies so they are comfortable rather than tight. A thin protective layer of barrier cream can shield the skin from moisture; avoid perfumed products and talc. Do not scrub, pick, or use steroid or antifungal cream unless a clinician has advised which product and how to use it.

Arrange medical advice if the rash is spreading, blistering, bleeding, crusting, unusually painful, or not settling after a few days of careful care. Seek prompt help if your child has fever, seems very unwell, is feeding poorly, or has fewer wet nappies. A clinician can check for infection, eczema, allergy, or another cause and recommend suitable treatment for the child's age.
