What to expect from vaginal oestrogen therapy for recurrent urinary tract infections

After menopause, reduced oestrogen can change the vaginal lining and the tissues near the urethra. Those changes may contribute to irritation and make urinary symptoms more troublesome. Local oestrogen is applied as a cream, tablet, pessary, or ring, according to the prescribed product.

This is prevention, not a treatment for an active infection. A clinician may need a urine test when symptoms begin, particularly if you have fever, blood in the urine, flank pain, or symptoms that do not fit your usual pattern. Vaginal oestrogen may take time to support tissue changes and does not replace antibiotics when an infection is diagnosed.

Follow the starting and maintenance schedule exactly, and ask what to do after a missed application. Wash your hands and use the applicator or ring as directed. Note burning, dryness, discharge, urgency, frequency, and episodes confirmed as infection; this helps distinguish irritation from a urinary infection at review.

Drink normally, avoid products that irritate the vulval area, and do not save or reuse antibiotics without medical advice. If symptoms recur, contact your care team early rather than assuming every episode is the same.

Seek prompt medical care for fever, chills, pain in the back or side, vomiting, visible blood in the urine, or feeling significantly unwell with urinary symptoms. Contact your prescriber about new vaginal bleeding, severe pelvic pain, or persistent irritation from the product. Difficulty passing urine also needs timely assessment.
