What to Expect from Vaginal Dilator Therapy for Vaginismus

Short answer
Expect a gradual, supported process in which a small dilator, lubrication and relaxed breathing help you practise control without forcing painful insertion.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why dilators may help
Vaginismus involves involuntary tightening of the muscles around the vaginal opening, which can make insertion painful or difficult. Dilator therapy offers a controlled way to notice muscle tension, practise relaxation and build confidence with your own pace. A pelvic health clinician may combine it with breathing work, pelvic floor treatment and support for fear or previous painful experiences.
Treatment usually begins with the smallest comfortable device. You may place it gently, pause while breathing slowly and remove it when the muscles relax. Moving up in size is optional and depends on comfort; the goal is not to complete a target size or tolerate pain.
Making practice more comfortable
Choose a calm private setting, wash your hands and device, and use the lubricant recommended by your clinician. Start with relaxation rather than insertion if your muscles are already tightening. Stop for sharp pain, panic, bleeding or worsening symptoms. Record what felt manageable and share it at follow-up so the plan can be adjusted. You remain in control of each session.
When to contact your care team
Arrange a review for ongoing pain, repeated bleeding, sores, unusual discharge or urinary burning. Seek urgent care for severe pelvic pain, heavy bleeding, fever or faintness. These symptoms may have another cause, such as infection or an injury, and should not be attributed to muscle tightening without an assessment.
Useful questions
Ask whether pelvic floor therapy or counselling should be part of your plan, which size to start with, how to respond to muscle spasm, and how progress will be judged beyond device size.

Questions people ask
Do I have to use a larger dilator?
No. Progress is based on comfort and your goals; your clinician can change the approach if a size feels difficult.
Can someone help during therapy?
A clinician can teach and support the technique, while home practice should remain consensual and under your control.
