What to expect from total hip replacement for avascular necrosis of the hip

Avascular necrosis damages the femoral head when its blood supply is reduced. As the bone weakens or changes shape, the hip can become painful and stiff and the joint surfaces may no longer move smoothly. Total hip replacement removes the damaged ball and replaces it with a stem and ball; the worn socket is fitted with an artificial lining.

After surgery, a physiotherapist helps you stand and walk safely, often with a frame or crutches. Recovery is gradual: swelling and fatigue may persist while the muscles regain strength. The approach used and the condition of the surrounding bone can affect restrictions and follow-up.

Arrange transport home, help with meals and a clear path around the house. Practise the exercises and walking technique provided by the team. Take medicines as prescribed, keep the incision clean and follow instructions about bending, twisting, sleeping positions and weight-bearing. Short, regular walks are usually more useful than pushing through a long walk, but your own plan takes priority.

Keep follow-up appointments so the wound, walking pattern and hip function can be checked. Tell the team about persistent pain, leg-length concerns or difficulty progressing with exercises.

Contact the surgical team for increasing wound redness, drainage, fever, worsening pain, or new difficulty putting weight through the leg. Seek urgent help for sudden breathlessness, chest pain, faintness, or marked calf swelling and pain. If the hip suddenly feels out of place after a twist or fall, stop walking and seek urgent assessment.

Ask which replacement approach is planned, what bone changes from avascular necrosis may alter the operation, and what movements to avoid. Clarify walking aids, wound care, driving, work, dental procedures, exercise and the schedule for follow-up imaging or review.
