What to Expect from Topical Hydroquinone Therapy for Melasma

Hydroquinone is a topical medicine that reduces pigment production in treated skin. It is usually applied as a thin layer to the melasma-affected areas, following the strength and schedule prescribed by a dermatologist. Keep it away from the eyes, lips, nostrils and broken skin unless your clinician gives different instructions.

The patches do not fade immediately. Improvement is gradual, and the response can vary with the depth of pigment, ongoing sun or visible-light exposure, hormonal influences and how consistently the medicine is used. Melasma can return after it improves, so a clinician may plan a break, maintenance care or another treatment approach rather than continuous use.

Apply only to clean, dry skin and wash your hands afterwards. Do not apply extra medicine to speed up fading; this can increase redness, stinging, peeling or dryness. Use a gentle cleanser and moisturiser, and protect the treated skin from daylight with broad-spectrum sunscreen, shade and a hat. Tell your prescriber about pregnancy, breastfeeding, eczema, allergies or other lightening products before starting.

Mild irritation can occur, especially early in treatment. If the skin becomes uncomfortable, contact the prescribing clinician before changing the schedule. Avoid adding scrubs, strong exfoliating acids or other irritating products unless they have been specifically included in your plan. Take a clear baseline photograph in the same lighting so changes can be discussed at follow-up.

Stop applying the product and seek prompt medical advice if you develop marked swelling, blistering, severe burning, a spreading rash or eye symptoms. Arrange a dermatology review for persistent redness, worsening irritation, new blue-black or grey discolouration, or darkening that continues despite stopping. These changes need assessment because prolonged or unsuitable use can injure or alter the colour of the skin.
