What to expect from topical corticosteroids for vitiligo

Vitiligo involves loss of pigment-producing cells in affected skin. A topical corticosteroid reduces local immune activity, which may allow repigmentation in selected patches. Response varies with the patch's location, duration, and activity. Facial skin and skin folds are more sensitive to steroid effects than thicker areas, so the product and schedule may differ. Repigmentation can appear uneven, and a patch may remain lighter even when treatment is helping.

Apply the medicine exactly as prescribed to the pale patches, avoiding normal skin unless told otherwise. Wash your hands after application and keep the product away from the eyes. Attend follow-up appointments so the clinician can inspect pigment change and skin texture. Protect depigmented areas from ultraviolet exposure with clothing and sunscreen recommended for your skin. Do not increase the amount or frequency when progress seems slow; that can increase irritation and thinning without making pigment return faster.

Contact your clinician if treated skin becomes very thin, shiny, bruised, persistently red, painful, or unusually hairy. Report eye-area symptoms, marked burning, or a rash beyond the application sites. Arrange review if patches spread quickly, new areas appear, or treatment causes distress, because the plan may need adjustment and other options may be considered.
