What to expect from topical corticosteroids for scalp psoriasis

Scalp psoriasis may form thick, flaky plaques that extend beyond the hairline and can itch or feel sore. A corticosteroid reduces inflammation where it is applied. As swelling and scale settle, the scalp may feel more comfortable, although the condition can flare again after treatment ends.

Solutions, foams, lotions and gels can spread through hair more easily than a thick ointment. Your clinician chooses the formulation and strength for the scalp and any nearby skin. Hair shedding from scratching can occur, but it is not the same as permanent hair loss.

Part the hair, place the product on the scalp rather than mainly on the hair, and massage gently if the instructions allow. Use the amount and schedule provided, then wash your hands. Do not pick or scrape firmly at adherent scale; ask whether a scale-softening product is appropriate.

Keep the medicine away from the eyes and face unless those areas were included in the instructions. Avoid improvised coverings or prolonged use. A comb, brush or pillowcase may retain product, so follow the product’s cleaning and washing directions.

Seek review if the scalp becomes increasingly tender, develops pus or crusting, or does not respond as expected. Urgent assessment is needed for facial swelling, breathing difficulty or a rapidly spreading severe rash. Report new patchy hair loss, broken hairs or scalp pain so infection and other causes can be considered.
