What to expect from topical corticosteroids for nummular eczema

Nummular eczema causes round or oval patches that may be dry, scaly, cracked, or weeping. A topical corticosteroid reduces the inflammatory activity responsible for itch and redness, so the patch may become less raised and uncomfortable before its colour fully returns to normal.

The prescriber selects strength and duration according to the patch, its location, and the skin's condition. Darker or lighter marks can remain after inflammation settles; this does not necessarily mean the eczema is still active.

Apply the prescribed amount in a thin, even layer to active patches, then moisturise as directed. Keep showers brief and use lukewarm water, choose a gentle cleanser, and avoid rubbing or scratching the plaques, since damaged skin can prolong the flare.

Use the medicine only for the planned course. Tell your clinician if several patches need treatment, if the skin is broken, or if you are using other steroid products. Covering a treated area can increase absorption and should be done only with advice.

Book a review if patches continue to enlarge, ooze, crust, or itch intensely despite treatment, or if new round patches keep appearing. Ringworm can resemble nummular eczema and needs a different treatment, so an uncertain diagnosis deserves examination.

Seek urgent care for rapidly spreading redness, severe pain, fever, facial swelling, or widespread blisters. These signs may indicate infection or a more serious reaction.

Ask how to distinguish an active patch from a leftover colour change, which moisturiser to use, and how long the steroid course should last. Ask whether a skin sample or fungal test is needed if the appearance is atypical or treatment does not help.
