What to expect from topical corticosteroids for heat rash

Heat rash, also called prickly heat or miliaria, develops when sweat cannot escape easily and the skin becomes irritated. A topical corticosteroid reduces local inflammation, so redness, swelling and itch may become less troublesome. It does not remove heat, moisture or friction, and it is not suitable for every rash that resembles heat rash. Thin, delicate areas and broken or infected skin need particular caution.

Use the prescribed strength and amount on clean, dry skin, following the clinician’s directions. Keep the area cool and dry with loose clothing, gentle washing and breathable fabrics. Avoid heavy ointments, tight coverings and vigorous scratching unless specifically advised. Wash your hands after application, unless the hands are being treated. If the rash is spreading, recurring or not settling, the diagnosis and treatment plan may need review.

Arrange prompt assessment for fever, increasing pain, pus, warmth that spreads, marked swelling or skin that is becoming raw. Seek urgent help for facial swelling, breathing difficulty or widespread hives. A rash that persists despite cooling, or one involving the eyes, mouth or genitals, deserves clinical review because another condition may need different treatment.

Ask which corticosteroid strength is intended, how often to apply it, how long to continue, and whether the product is appropriate for the affected body site. Also ask how to recognise infection and what cooling measures can prevent another flare.
