What to expect from topical calcineurin inhibitors for morphea

Morphea, also called localised scleroderma, causes areas of skin to become firm, thickened, discoloured, or tight. A topical calcineurin inhibitor reduces local immune activity without being a steroid. A dermatologist may consider it for superficial or active patches, while deeper, widespread, fast-changing, or function-limiting disease may need a broader treatment plan.

The skin may not look different quickly. Activity can be followed by changes in colour, a lilac-coloured edge, firmness, or spread. Burning or warmth after application can occur, especially on inflamed skin, and should be discussed if it continues.

Apply a thin layer to the prescribed area on clean, dry skin and wash your hands afterward. Avoid eyes, mucosal surfaces, and broken skin unless your clinician specifically directs otherwise. Use an unperfumed moisturiser if advised, and avoid vigorous rubbing over a firm or tender patch.

Keep appointments because the key question is whether morphea is active, not only whether the surface feels smoother. Note new patches, increasing tightness, reduced movement near a joint, or changes in hair over the area. Do not stop or add medicines without discussing the plan.

Arrange an earlier review if a patch expands, becomes more indurated, develops an active coloured border, or restricts movement. Contact your clinician for persistent severe irritation, cracking, or signs of infection. Urgent help is needed for facial swelling, breathing difficulty, or a widespread blistering reaction.
