What to expect from topical calcineurin inhibitors for inverse psoriasis

Inverse psoriasis affects folds such as the groin, armpits, under-breast area, or cleft between the buttocks. The skin may look smooth, shiny, red, or darker rather than scaly because moisture and friction change the fold. A calcineurin inhibitor is a non-steroid anti-inflammatory applied to the affected area. It may be selected when a clinician wants to limit repeated steroid exposure on thin or sensitive skin.

A warm, tingling, or stinging feeling can happen when treatment starts, particularly on irritated skin. Improvement may be noticed as less redness, soreness, or itching, but flares can return if triggers such as friction, sweat, or infection remain.

Use a small amount on clean, thoroughly dry skin according to the prescribed schedule. Wash your hands after applying it and avoid covering the fold with an airtight dressing unless instructed. Pat folds dry after washing, choose breathable clothing, and reduce rubbing where possible.

Do not assume every fold rash is psoriasis. Yeast, bacterial infection, eczema, and contact reactions can look similar and may need different treatment. Arrange follow-up if the area is not settling, develops an unusual odour or discharge, or becomes increasingly tender.

Ask for prompt medical review if pain, warmth, swelling, pus, fever, or rapidly worsening redness develops, because infection can occur in a moist skin fold. Seek urgent help for facial swelling, breathing difficulty, or a widespread blistering reaction. Contact your clinician if burning persists or the medicine seems to worsen the rash.
