What to Expect from Topical Calcineurin Inhibitors for Cutaneous Lupus

Cutaneous lupus affects the skin and may cause persistent patches, plaques, scale, sensitivity, or changes in colour and texture. A topical calcineurin inhibitor dampens local immune activity. It may be chosen for certain areas or when a clinician wants a non-steroid option, but it is not suitable for every lesion or every pattern of lupus.

Inflammation may become quieter before marks fade. Some lesions can leave lasting changes, so early assessment and consistent sun protection matter. Treatment for skin lupus may sit alongside other medicines; do not change those medicines based only on how one patch looks.

Apply a thin layer only to the areas prescribed, on clean and dry skin. Keep it away from the eyes, lips, and broken skin unless your clinician gives specific instructions. Wash hands after application when the hands are not being treated, and avoid tight coverings unless directed.

Protect exposed areas from sunlight with protective clothing and a suitable sunscreen, because ultraviolet exposure can aggravate cutaneous lupus. Avoid picking, abrasive scrubs, and fragranced products on active lesions. Keep a simple record of new spots, scale, tenderness, or hair changes for follow-up.

Contact your clinician if a treated area becomes increasingly painful, swollen, blistered, crusted, or infected-looking. Arrange prompt review for rapidly spreading lesions, new scarring or hair loss, sores that do not heal, or symptoms beyond the skin such as unusual joint pain, chest discomfort, or marked fatigue. These features may require broader assessment.
