What to expect from topical antifungal treatment for nappy rash

Some nappy rashes are caused or worsened by Candida yeast in the warm, damp nappy area. An antifungal medicine targets that yeast, while the skin also needs protection from urine, stool, moisture, and rubbing. The redness may not disappear immediately, and a tender surface can take longer to recover. Because several types of rash resemble one another, a clinician may need to confirm that an antifungal is appropriate.

Change nappies promptly and clean the area gently with lukewarm water or a suitable fragrance-free cleanser. Pat dry, allow brief air exposure when practical, then apply the antifungal exactly as directed. Use a clean hand for each application and wash it afterward. A clinician or pharmacist can advise whether a barrier ointment should be layered with the medicine. Avoid talc, fragranced wipes, and rubbing the rash, which can add irritation.

Seek medical advice promptly if the baby seems unwell, has a fever, develops blisters, pus, open sores, marked swelling, or rapidly worsening pain. Arrange review if the rash is not improving with correct treatment, keeps coming back, or spreads beyond the nappy area. Very young babies and children with other health conditions may need earlier assessment.
