What to expect from surgical sperm retrieval for male factor infertility

Surgical sperm retrieval is used when sperm are absent from semen, blocked from reaching the semen, or difficult to obtain with ejaculation. A clinician takes a small sample from the epididymis or testicle using the approach that fits the suspected cause. The sample is examined by a laboratory, and usable sperm may be used immediately or prepared for freezing and later assisted reproduction.

The procedure may be performed with local anaesthetic, sedation, or general anaesthesia. Your fertility team will coordinate the timing with egg collection if the sperm is intended for an assisted reproduction cycle. Finding sperm is not assured, because the result depends on sperm production and the underlying condition.

Before the procedure, tell the team about medicines, allergies, bleeding problems, and previous scrotal surgery. Follow the instructions about fasting, bathing, and arranging transport if sedation or anaesthesia is planned. Wear supportive underwear and ask whether a semen sample or partner treatment needs to be scheduled on the same day.

Afterward, mild swelling, bruising, tenderness, or a small amount of wound drainage can occur. Rest, use pain relief exactly as advised, and apply cold packs with a cloth barrier if recommended. Keep the wound clean and dry according to the discharge instructions. Avoid strenuous activity, heavy lifting, cycling, and sex until your clinician says they are appropriate. Attend the follow-up appointment so the result and next fertility step can be discussed.

Contact your clinic promptly if pain or swelling is worsening rather than settling, the dressing becomes repeatedly soaked with blood, the incision opens, or you develop fever, spreading redness, pus, or a new bad smell. Seek urgent care for severe pain, rapidly increasing scrotal swelling, fainting, chest pain, or trouble breathing. Use the discharge number if you are unsure whether a symptom is expected.

Ask which retrieval method is recommended and why, what anaesthesia will be used, how the sample will be handled, and when results will be available. Clarify the recovery restrictions, the plan if no sperm is found, and whether sperm freezing or assisted reproduction will be arranged.
