What to expect from picosecond laser treatment for freckles

Freckles are small, flat areas of increased pigment that often become more noticeable after sun exposure. A picosecond laser delivers very short energy pulses to the pigment. You may feel a snapping or prickling sensation, followed by warmth. Eye protection is used, and cooling may be offered. Settings are selected according to the colour and size of the freckles, their location, and your skin tone.

Immediately afterwards, the skin may be pink, swollen, or temporarily darker. Fine crusting or a peppered appearance can occur as pigment is shed. Freckles may fade but can return with later sun exposure, and not every pigmented mark is a freckle. A clinician should examine any spot that behaves differently from the others.

Tell the clinician about recent tanning, medicines, a tendency to form raised scars, cold sores, and previous pigment changes after skin injury. Avoid intentional tanning before and after treatment. Keep treated skin clean, avoid picking or scrubbing, and use the dressing or gentle product recommended for you.

Once advised, apply suitable sun protection every day and use a hat or clothing when practical. Avoid exfoliating acids, retinoids, fragranced products, and heat exposure until irritation has settled or the clinic says they are appropriate. Keep a follow-up if one is arranged, especially when a mark fades unevenly or remains unlike the surrounding freckles.

Ask the clinic for help if redness spreads, pain increases, swelling becomes marked, blisters are extensive, or the skin develops pus, an unpleasant smell, or fever. Seek urgent care for swelling that affects breathing or vision. Have any freckle-like mark reviewed promptly if it enlarges, changes shape or colour, bleeds, crusts, or does not heal, rather than assuming it is a treatment reaction.
