What to expect from physiotherapy for chondromalacia patellae

Short answer
Expect an assessment of knee movement followed by exercises for the thigh, hip and movement patterns that load the kneecap.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Muscles often trained
- Quadriceps, hip and trunk muscles
- Activity examples
- Stairs, squats, walking, running and landing tasks
What the assessment looks at
Your physiotherapist may ask when pain appears, such as during stairs, squatting, running or sitting with the knee bent. They can observe kneecap movement, leg alignment, hip control, ankle movement and the strength of the quadriceps and surrounding muscles. Early treatment may use gentle strengthening and activity changes; later work can add step-downs, balance tasks and sport-specific movement when tolerated.
How to support recovery
Follow the exercise plan at a manageable level and increase repetitions, resistance or activity only as advised. Temporarily reduce movements that repeatedly provoke pain, while keeping comfortable activity such as walking if it is suitable for you. Use the therapist’s technique cues for stairs, squats and landing. Pain that lingers or swelling that increases should be discussed before progressing the programme.
When to seek medical care
Seek review if the knee repeatedly gives way, locks, becomes markedly swollen, or cannot bear weight. Prompt assessment is also sensible after a new injury with severe pain or a visible change in shape. A hot, red knee with fever needs urgent medical attention. These signs may require more than routine rehabilitation.
Questions to ask
Ask which activities to modify, how much discomfort is acceptable during exercise, when to add resistance, and how hip or foot mechanics affect your knee plan.

Questions people ask
Will physiotherapy involve strengthening the knee?
Usually it includes progressive quadriceps and hip strengthening, selected to improve control without repeatedly aggravating kneecap pain.
Do I need to avoid stairs completely?
Not necessarily. Your therapist can help modify stair technique and volume while building the strength needed for this activity.
