An internal examination is not automatic; it should be explained and agreed to, and alternatives are available.

Pain can involve muscles that are overactive or poorly coordinated, not only muscles that are weak. Treatment therefore focuses on reducing sensitivity and improving movement and control rather than forcing a strong squeeze.

What sessions and home care may include

Sessions may use diaphragmatic breathing, gentle stretches, graded movement, relaxation training, education about pain, and carefully paced activity. If appropriate, hands-on treatment can address tender muscles or connective tissues, with you able to pause or stop at any time.

Follow the home plan at a tolerable level and track pain, sleep, activity, bladder, and bowel changes. A flare does not always mean harm, but a sustained increase should be reported so the plan can be adjusted.

When to seek medical advice

Tell your physiotherapist or doctor if treatment repeatedly increases pain, causes bleeding, or brings new urinary or bowel difficulty. Seek urgent care for sudden severe pelvic pain, fainting, heavy bleeding, fever with pelvic pain, or inability to pass urine.

Ask what may be contributing to your pain, whether an internal assessment is useful, and what alternatives exist. Clarify the home exercises, signs to reduce activity, and how other causes of pelvic pain will be evaluated.
