What to expect from paediatric physiotherapy for juvenile idiopathic arthritis

Short answer

Expect therapy to focus on comfortable joint movement, strength, energy management and participation, with the programme changing during flares and quieter periods.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Team care
Physiotherapy works alongside rheumatology treatment.

What sessions may include

Juvenile idiopathic arthritis can cause joint swelling, stiffness and reduced movement. A paediatric physiotherapist may assess joints, muscle strength, flexibility, walking, posture and how symptoms affect play, school and self-care. Treatment may include gentle range-of-motion work, strengthening, balance practice and advice about pacing. The plan should complement the child’s rheumatology care; physiotherapy does not replace prescribed medicines or medical monitoring.

Supporting the plan at home

Ask the therapist to show each exercise and agree how discomfort should be judged. Short, regular practice may be easier than one demanding session. Encourage warm-up, rest breaks and activities that keep joints moving without provoking a flare. During increased swelling or morning stiffness, contact the clinical team about modifying the plan rather than pushing through. Share updates from school, sports and home so goals remain practical.

When to seek medical advice

Contact the child’s medical team for a new or worsening swollen joint, persistent pain, a new limp, marked loss of movement or symptoms that interfere with sleep or ordinary activities. Seek urgent care for a hot, very painful joint with fever, sudden inability to use a limb or a child who appears seriously unwell. These changes may need medical assessment before therapy continues.

Questions to ask

Ask which joints need protection, how to recognise a flare, and which exercises are suitable during active symptoms. Clarify pain and fatigue limits, school and sports adjustments, home equipment, and how the physiotherapist and rheumatology team will share progress.
